Waterford Hockey Club Members!
In accordance with Sport Ireland and national health advice, we are required to ask all who intend to train, coach or assist with the club’s on-field activities to fill in a brief health questionnaire prior to entering the pitch at Newtown Link to questionnaire: https://docs.google.com/forms/d/1RKbE0BL3WJAGvP9kddLMba708Sk4jP-fvJInxAGNBRA/edit
We are asking everyone to complete this by Wednesday, August 5th so that our COVID-19 Officer and Supervisors have a complete list of all training squads for tracking and tracing going forward.
If you are unsure which squad you will be training for, or if you intend to train with more than one squad, we ask you to fill in a form for each squad.
I should point out that this is separate from your Club Membership Form, which will also be requested in due course.
I’m sure you are aware of the volume of administration required to comply with the COVID protocols and we thank you for your patience.
Unfortunately, we cannot allow anyone to enter the pitch in Newtown without first having submitted this health questionnaire.
